The goals of the Coagulation Service are to:
- Provide rapid and accurate diagnosis of bleeding and clotting disorders.
- Initiate appropriate therapy for patients with bleeding and clotting disorders.
- Minimize delays for surgical procedures in patients with hemostatic defects.
- Promote the efficient utilization of laboratory and blood resources.
- Facilitate early hospital discharge by assisting in anticoagulant management.
Attending physicians on this service have significant clinical and research interests in the area
of hemostasis and thrombosis and are available on a 24 hour basis. The close integration of the
Duke Coagulation Consult Service with the Duke Clinical Coagulation Laboratory allows us to provide
a number of unique services:
Diagnosis and treatment of patients with bleeding disorders:
- History of abnormal bleeding
- Prolonged bleeding or clotting times
- Factor deficiencies
- Thrombocytopenia
- Complex coagulopathies
- Therapeutic use of coagulation factor concentrates
- Expedited laboratory evaluations when clinically indicated
Diagnosis and treatment of patients with recurrent thrombosis:
- Recurrent venous thrombosis (DVT or PE)
- Recurrent arterial thrombosis
- Young onset of DVT or PE without predisposing factors
- Lupus anticoagulants and anticardiolipin antibodies
- Deficiencies of natural anticoagulants (Protein C, Protein S, Antithrombin III)
- Genetic and functional testing for Activated Protein C Resistance
Diagnosis and management of heaparin induced thrombocytopenia (HIT):
- Expedited laboratory testing and interpretation
- Research Protocols for alternative anticoagulants
Anticoagulation Clinic:
- Recommendations for duration and intensity of anticoagulation
- Stabilization of oral anticoagulation following hospital discharge
- Management of oral anticoagulation in routine and high risk patients
- Outpatient Heparin Therapy
Management of coagulation disorders in surgical patients:
- Same day preoperative evaluation of patients with abnormal bleeding history or screening tests (both inpatient and outpatient)
- Perioperative management of anticoagulants in patients with a high risk of thrombosis
- Diagnosis and management of postoperative bleeding
